Differentiating incisional hernia from Spigelian hernia in a patient with previous abdominal surgery case report

Abstract: Incisional hernia and Spigelian hernia are two types of abdominal wall hernias that may exhibit comparable symptoms, complicating the diagnostic process. Spigelian hernias occur via the spigelian fascia, whereas Incisional hernias arise through a prior surgical incision or scar. This report describes a 52-year-old obese patient with a history of supraumbilical hernia repair and appendicectomy, presenting with swelling at the appendectomy scar site. Physical examination showed reducible, non-painful swelling, while a CT scan revealed a 12x12 cm defect in the spigelian fascia, with herniated loops of small bowel and mesentery. Despite the diagnostic dilemma, an open mesh repair was successfully performed. The patient was discharged on the third day. This case underscores the diagnostic difficulty in distinguishing Incisional from Spigelian hernias, especially in the presence of a surgical scar in the Spigelian belt region. An in-depth comprehension of the characteristics of both hernias is crucial for achieving optimal patient outcomes.
