Resilient reconstruction: A case report on cartilage tympanoplasty and its efficacy in retracted tympanic membrane

Abstract: Cartilage tympanoplasty offers significant structural benefits in reconstructive middle ear surgeries, particularly in patients with atelectatic tympanic membranes. This case report presents a 57-year-old male with progressive left ear hearing loss and a grade IV retracted tympanic membrane. Diagnostic evaluations confirmed a conductive hearing loss of 47 dB and normal ossicular integrity. Surgical intervention involved tympanoplasty combined with a novel technique—boomerang ossiculoplasty—using a chonchal cartilage graft. Sculpted into a boomerang shape, the graft provided both structural support and effective sound conduction, with one end placed on the stapes footplate and the other in the hypotympanum. At six months postoperatively, the patient demonstrated marked hearing improvement with air-bone gap closure to within 10 dB and stable tympanic membrane positioning without recurrence of retraction. This innovative approach not only addresses the structural limitations posed by chronic Eustachian tube dysfunction but also enhances acoustic outcomes by reinforcing ossicular continuity. Boomerang ossiculoplasty represents a promising surgical technique for long-term auditory restoration in retracted tympanic membranes and related middle ear conditions. Further research with larger cohorts is recommended to validate these findings and refine surgical protocols.
