Abstract: Nasolabial cysts are rare, non-odontogenic, soft tissue cysts that develop in the nasolabial fold, often leading to facial asymmetry and nasal obstruction. Diagnosis is based on clinical evaluation, supplemented by imaging techniques like Orthopantomogram (OPG), Computed tomography (CT) or Magnetic resonance imaging (MRI) and to confirm the cystic nature and differentiate it from other maxillofacial lesions. Surgical excision, particularly via a sublabial approach, is the preferred treatment to ensure complete removal and prevent recurrence. This report presents a middle-aged female with progressive swelling on the left side of the face and nasal obstruction. Clinical examination revealed a well-defined, non-tender swelling in the nasal vestibule, affecting the nasolabial fold and nasal airway. CT imaging confirmed an extra-osseous cystic lesion adjacent to the nasal ala, without signs of bony involvement. The patient underwent sublabial enucleation under general anesthesia, leading to successful removal of the lesion without postoperative complications.Following surgery, the patient experienced complete resolution of symptoms, with histopathology confirming a cyst lined by pseudostratified columnar epithelium, consistent with a nasolacrimal duct origin. This case highlights the importance of early diagnosis and surgical intervention to prevent complications such as infection, recurrence, and nasal and facial deformities.
