Abstract: Mucoepidermoid carcinoma is the most common malignant tumor of the salivary gland across all ages. Mucoepidermoid carcinoma typically presents as a painless mass, with continuous growing, it is progression ranges from quiescent to highly invasive with metastasis, mucoepidermoid carcinoma can invade locally, and is also more likely to have nodal metastasis depending on the histological subtype, One of the most important factors affecting prognosis and treatment is the presence of metastasis. Metastases are indicators of poor prognosis and commonly occur in the lymph nodes, lungs, bones, and brain. Certain subsets exhibit aggressive behavior regardless of tumor histological grading, such as the submandibular salivary gland, which has a prognosis similar to high-grade tumors, as well as the tongue, and floor of the mouth. Mucoepidermoid carcinoma tends to metastasis through lymphatic routes. However, in this case there was intravenous metastasis without direct invasion of the vein, which is an unusual presentation of metastasis for the mucoepidermoid carcinoma of the submandibular salivary gland. This may give a clue about the unpredictable behavior of these tumors which may influence the treatment choice and prognosis of the treatment.
