Abstract: Reconstructing defects after excision of carcinoma of the external auditory canal is a challenging undertaking. Factors such as functional, aesthetic, and technical difficulties, as well as donor site morbidities and the ability to withstand radiotherapy, must be considered during the reconstruction process. The need for a local flap that can address these requirements is uncommon. However, a bilobed local transposition flap can be a suitable choice, as it can satisfy the aforementioned needs. This flap can be executed with a predictable outcome and may also be utilised for post-traumatic defects. Additionally, we describe a modification in the closure of the secondary defect to overcome technical complexities.
