Endometrial adenocarcinoma causing intrauterine fetal death and uterine rupture: A case report

Abstract: Endometrial adenocarcinoma is the second most common gyneacological cancer. It commonly occurs in postmenopausal women but 12% of patients are premenopausal. Endometrial adenocarcinoma during pregnancy is rare and extremely rare after delivery. We report the case of a 46 years old female gravida 4, para 4, obese (BMI= 31kg/m2). She was followed up for a pregancy with no abnormality in ultrandsound. In May, 10th 2023, she was diagnosed with fetal death in utero at 39th gestational week and a labor induction was performed. Two days after delivery, she was admitted in emergency for abdominal pain. At clinical examination, she had cardiovascular collapse and peritoneal irritation signs. The abdominal CT scan showed a hemopeeritoneum of great abundance with uterus rupture; a subtotal hysterectomy and bilateral salpingo-oophorectomy were performed in emergency. The histological examination of the piece showed an endometroid adenocarcinoma well differencied grade 1 with myometrial invasion (pT1bNxMx).No metastasis was found at pelvic MRI and CT TAP. Three month after the surgery was completed with trachelectomy and pelvic lymphadenectomy. The histological examination of surgical specimen showed CIN 1 on condyloma with no tumoral cell and no lymph node affected. The multidisciplinary team meeting decided a surveillance. There is no recurrence after eighteen months of follow up.
