Abstract: Ductal adenocarcinoma of the sublingual gland is a rare and aggressive malignancy, characterized by its tendency for early metastasis and local invasion. This case report presents an advanced case of ductal adenocarcinoma of the sublingual gland with extensive bone invasion, highlighting the clinical presentation, diagnostic challenges, and therapeutic approach. A 65-year-old male patient presented with a painful, progressively enlarging mass under the tongue, accompanied by difficulty in swallowing and speaking. Imaging studies revealed extensive bone involvement, making surgical resection challenging. The patient underwent a combination of surgical intervention and adjuvant therapy, with subsequent follow-up to monitor for recurrence. This case emphasizes the importance of early recognition and multidisciplinary management in cases of advanced ductal adenocarcinoma of the sublingual gland, as well as the need for further research to optimize treatment protocols for such rare and aggressive tumors.
