Abstract: Adnexal torsion is a rare condition characterized by the complete or partial rotation of the adnexa around its vascular axis. Its occurrence in a healthy ovary is uncommon, and the diagnosis is often challenging due to the condition's nonspecific presentation, which complicates differential diagnosis. We present a case of adnexal torsion in a 20-year-old woman who arrived with acute abdominal pain. Initially, she was misdiagnosed with a hemorrhagic cyst, leading to a delay in her referral to a gynecologist. Emergency laparoscopy and adnexal detorsion were eventually performed. This case underscores the diagnostic challenges and the critical importance of timely, appropriate therapeutic decisions.
