Abstract: Non-traumatic gallbladder perforation is a rare and insidious complication of cholecystitis, which may lead to mortality. Formation of a subsequent liver abscess further necessitates prompt diagnosis and management. There is difficulty in differentiating such disease from regular cholecystitis. A sensitive abdominal ultrasound and computerized tomography scan provide accurate preoperative diagnosis. In cases involving marked distortion of hepatobiliary anatomy and large hepatic abscess, open cholecystectomy is a more favorable option along with preliminary conservative management, though length of hospital stay is longer than laparoscopic cholecystectomy.
